Local Authorities

Adult social care teams, commissioning teams and care managers can refer directly. We work closely with local authorities across London and beyond.

Healthcare

NHS & CMHTs

Community Mental Health Teams, community nurses and hospital discharge teams. We accept referrals from NHS partners and work collaboratively on care planning.

Professionals

Social Workers

Social workers and care co-ordinators can refer individuals they are supporting. We respond quickly and keep you updated throughout the assessment process.

Family

Families & Carers

Families and carers who are concerned about a loved one are welcome to contact us directly. We will guide you through the referral and assessment process with care.

Individual

Self-Referrals

Individuals who feel they would benefit from supported living can contact us directly. We take self-referrals seriously and assess each person on their own terms.

Commissioners

Commissioning managers and framework managers can refer and commission placements directly. We are experienced in working within commissioning frameworks.

The Referral Process

From First Contact to First Home.

Same day

Get in Touch

Contact us by phone, email or online form. We acknowledge every referral the same day and assign a named contact for all communication.

24 hours

Initial Screening

We review the referral to establish suitability and whether the person's needs can be safely met within our supported living model.

Person-centred care planning with Majestic Homes
3–5 days

Full Assessment

A comprehensive needs assessment covering history, current needs, communication preferences, risks and personal goals.

1–2 weeks

Care Plan Co-Production

The care plan is built collaboratively with the individual, family and professionals — reflecting real wishes, not a generic template.

Agreed date

Placement Confirmed

A suitable flat is matched to the person. Move-in is planned collaboratively with the individual and all key parties involved in their care.

Monthly

Ongoing Review

Support is reviewed regularly — monthly at minimum. Plans are updated to reflect growth, changing needs and the person's evolving goals.

What Happens Next

After You Submit Your Referral.

Named Contact Assigned

You will be given a named contact at Majestic Homes who handles your referral from start to finish — no being passed around departments.

Information Requested

We will ask for any existing assessments, care plans, health information or risk assessments that will help us understand the person's needs.

Assessment Scheduled

We arrange a face-to-face or virtual assessment with the individual (and their support network where appropriate) at a time that suits them.

Placement Decision Made

We confirm whether we can meet the person's needs, identify the right flat, and agree a move-in timeline with you and the individual.

Support Begins

On move-in day, a keyworker is introduced, routines are established and the care plan is active — the person's journey begins on day one.
